Our Commitment
Father Lacombe Care Centre is committed to protecting the privacy of the personal information of its residents, their families, our employees, and volunteers. We value the trust of those in our care and those with whom we interact on a business and personal basis. Our core values of excellence, compassion, spirituality, sacredness of life, and justice commit us to maintaining a high level of integrity and accountability in all our practices.
In order to enable the day-to-day services and level of care we provide we must collect specific personal information, including health information for residents. Our privacy practices are designed to ensure such information is collected, used, and disclosed on a need-to-know basis, and retained and disposed in a secure manner.
What is Personal Information?
Personal information is any information that can be used to distinguish, identify, or contact a specific individual. This information can include an individual's opinions or beliefs, as well as facts about, or related to, the individuals. Exceptions are business contact information and certain publicly available information, such as names, addresses and telephone numbers as published in telephone or other directories.
Where an individual uses his or her home contact information as business contact information as well, we consider that the contact information provided is business contact information, and is not therefore subject to protection as personal information.
Our Privacy Practices
Father Lacombe Care Centre is authorized by the Health Information Act to collect, use, and disclose personal health information to facilitate medical service for individuals in our care.
In accordance with that legislation, as well as the Freedom of Information and the Protection of Privacy Act, the provincial Personal Information Protection Act and federal Personal InformationProtection and Electronic Documents Act, all personal information provided to our business office is collected, used and disclosed on a need-to-know basis and only for the purposes relating to the provision of health services. No information may be collected, used, or disclosed for purposes other than the provision of health services, without expressed consent.
As a not-for-profit organization, Father Lacombe care Centre must actively engage in fundraising activities to address the non-funded capital needs of our clients and residents. Written consent is required before the information provided to the business office can be transferred to the development office for fundraising purposes. Consent may be withdrawn at any time by notifying Father Lacombe Care Centre in writing. We collect and use personal information only for purposes that a reasonable person would consider appropriate to the circumstances. The personal information of all donors is maintained confidentially and is not sold or shared. Father Lacombe Care Centre ascribes to the Donor Bill of Rights developed in part by the Association of Fundraising Professionals.
We routinely offer individuals the opportunity not to have their information shared for purposes beyond those for which it was explicitly collected.
We endeavor to ensure that information is accurate, complete, and up-to-date. Individuals have the right to request access to their personal information, and request amendment as appropriate.
We use password protocols and encryption software to protect personal and other information we receive. Our software is routinely updated to maximize protection of such information.
All reasonable efforts have been made to protect the security of personal information collected by Father Lacombe Care Centre, and information is securely disposed after a minimum of seven years.
Individuals have the right to challenge Father Lacombe Care Centre compliance to our privacy codes by contacting the Privacy Officer in writing.
Updating of Privacy Policy
We regularly review our privacy practices for our various activities and update our policy as required.
